IT Survey Questionnaire
- I. Physical Assessment
We will directly assess the following points.
- 1. The computers:
(a) Does the site have computers?
(b) How many?
(c) What type(s) (CPU, CD-ROM, HDD, RAM)?
(d) What computer accessories (printer, scanner, extra parts)?
(e) How many are set up?
(f) How many function?
(g) Are they networked?
(h) Do the computers have Internet access?
(i) Other comments?
- 2. The power:
(a) What type of electricity is there (solar, generator, city, combination, other)?
(b) What is the capacity of the electricity (watts, amps)?
(c) Is there a stabilizer or UPS (list capacity)?
(d) Additional comments on schedule (for city power) and quality of electrical source?
- 3. The facility:
(a) Is the room a permanent or temporary computer space?
(b) Is the room secure?
i. Does the room have a good lock?
ii. Are there bars in the windows?
(c) Is there a method of cooling the room (AC or fans)?
(d) Does the roof appear to leak?
(e) Will dust likely be a problem?
(f) Are computers covered?
(g) Do the windows have curtains?
(h) Other comments?
- 4. Any additional observations?
If appropriate, the site will be photographed.
- II. Interview of the Steward of the Site
We will interview one appropriate staff member for each site. Appropriate staff members could include the primary computer user (ie a secretary in an office), computer lab managers, or senior administration officers.
- II-A Questions for sites with computers
1. What is your name and position?
2. How many working computers does your institution have?
3. How many non-working computers?
4. Where did these computers come from and when did your institution get them? (Please account for each computer if possible.)
5. Who has access to these computers?
6. Who has used them in the past year?
7. What are they used for? (secretarial, student teaching, staff teaching, community teaching, income generation, Internet, other)
8. What people at your institution do you consider qualified to use computers (and how many total)?
9. What people at your institution do you consider qualified to teach computers (and how many total)?
10. Who maintains computers?
11. Where do you obtain parts?
12. List your two most common problems. (software, printer, modem/Internet, computer not starting, electrical, other (specify))
- II-B Questions for sites with no computers
1. What is your name and position?
2. Has your institution ever had computers in the past? If yes, then
(a) When did the institution get these computers and where did they come from?
(b) Why are they no longer available?
3. What people at your institution do you consider qualified to use computers (and how many total)?
